Tamara N. Thomas

VP, Population Health Impact
Aledade · Washington, D.C.

Tamara N. Thomas is a national healthcare strategist, operator, and emerging thought leader who has spent over two decades proving that health equity and operational excellence are not competing priorities — they are the same priority.

As Vice President of Population Health Impact at Aledade, Tamara leads the integration of health equity and population health across one of the nation’s largest primary care networks, serving over two million patients. Her work is anchored by four convictions she has made distinctly her own: that health equity must be operationalized through data, not just declared in mission statements; scalable impact requires building true ecosystems of care across diverse stakeholders; that AI’s greatest potential in healthcare is as a tool for deepening human empathy rather than replacing it; and that patient experience is the ultimate measure of whether any of it is actually working.

“Technology is most powerful when it’s an empathy enabler — not a replacement for the relationship between a provider and their patient.”

What sets Tamara apart is her ability to “hardwire” evidence-based strategy into the day-to-day realities of clinical and business operations. She is not a theorist. She is an architect. Her track record includes structuring cross-sector partnerships that delivered a 2.7x ROI with national payers, leading population health initiatives in some of the country’s most underserved rural communities, and advancing workforce and hiring strategies that center representation as a clinical and operational imperative, not an afterthought.

Over the course of her career, Tamara has become a trusted voice on the social drivers that shape community health outcomes — and a practitioner who understands that sustainable change requires building resilient systems, not just launching programs. Her expertise in rural health complexities, value-based care, and SDOH integration has made her a sought-after speaker, advisor, and strategic partner for health systems, payers, and mission-driven organizations navigating the pressures of a rapidly evolving landscape.

Beyond her institutional work, Tamara is committed to the kind of ecosystem building that lifts the entire field. She actively contributes to the industry’s most consequential dialogues through her involvement in the Health Care Transformation Task Force and the Health Care Payment Learning & Action Network (HCPLAN), where she contributes to the Patient Empowerment Workgroup and helps shape payment models that prioritize patient-centered outcomes.

She believes that the next generation of healthcare leaders — particularly those from underrepresented communities — deserve to see themselves at tables where the most consequential decisions are being made. That belief shapes how she leads, mentors, and shows up in every room she enters.
